In order to meet these growing needs, the Chanda Plan Foundation has now developed a new vision for the future – a Patient Centered Medical Home (PCMH), where disability is the norm and all of our services will be available under one roof for better health access, coordination, and outcomes. The Chanda Plan Foundation’s on a path to purchase a 12,000 square foot facility that can offer hundreds of treatments per week, and serve as a “one-stop-shop” for the health needs of people with long-term physical disabilities. This holistic and person-centered approach will include a primary care physician, nutrition and a counseling in addition to current services of adaptive yoga, massage, and acupuncture.


The Chanda Plan Foundation is the only one of its kind in the United States and is establishing a delivery of healthcare for persons with disabilities that is long overdue. As we follow the trends of the healthcare system, the overuse of costly interventions, and the growing number of people that need our services, we keep our sights on the larger vision.
